normal 0 false false false en-us x-none ar-sa microsoftinternetexplorer4 background : it has been shown that bispectral index (bis) may reduce during spinal anesthesia. the aim of this study was comparing bis scores during spinal anesthesia using hyperbaric bupivacaine alone or with intrathecal or intravenous fentanyl for cesarean section. methods : in this double-blind randomized controlled clinical trial, 140 pregnant women with american society of anesthesiologists (asa) class i or ii were randomly assigned to receive 1-2.5 ml hyperbaric 0.5%   plus normal saline (b), 2-2.5 ml hyperbaric 0.5%   plus 20 µg fentanyl intrathecally (bf it ), or 2.5 ml hyperbaric 0.5%   plus 100 µg fentanyl intravenously (bf iv ). bis was measured using electroencephalography (eeg) findings and recorded at the baseline and 5 th , 10 th , 15 th , 30 th, , 45 th , and 60 th minutes after spinal injection. the heart rate (hr), mean arterial pressure (map), signal quality index (sqi), and temperature were also recorded during surgery. results : a total number of 140 subjects completed the study and underwent analysis. the recorded bis was significantly different between the three groups at all time points (p = 0.004). at all the times, the bis values were lower in the bf iv group compared to other groups (p < 0.05). the bis scores in group b were significantly higher in most time points compared to other groups. after spinal anesthesia, the changing trend of bis reduced until the 30 th minute for the bf it group and until the 45 th minute for the b and bf iv groups. conclusions : although the bis scores began to decrease during spinal anesthesia using hyperbaric bupivacaine alone or with intrathecal or intravenous fentanyl for cesarean section, the greatest reduction from baseline bis values occurred with adding intravenous fentanyl. moreover, maximum reductions of bis scores appeared at 30 and 45 minutes after induction of spinal anesthesia. keywords : anesthesia; bispectral index; depth; spinal

The effects of hyperbaric or isobaric bupivacaine on bispectral index in spinal anesthesia for cesarean section

BACKGROUND Hyperbaric and isobaric bupivacaine has been shown to reduce bispectral index (BIS) during spinal anesthesia. The aim of this study was to compare the effect of isobaric bupivacaine and hyperbaric bupivacaine on the BIS during cesarean section with spinal anesthesia. MATERIALS AND METHODS In this double-blind randomized controlled clinical trial, 84 pregnant women with ASA (America...

INCIDENCE OF SHIVERING DURING CESAREAN SECTION IN PATIENTS UNDER SPINAL ANESTHESIA WITH OR WITHOUT FENTANYL

Background & Aims: Postoperative shivering is one of the common complications of surgery in the recovery room, with an incidence rate of about 50-60% in various reports. Postoperative chills usually result in unwanted hypothermia during operation. Materials & Methods: In this study, patients were divided randomly (after selecting one of 32 card containing coatings) into intervention group (n=4...

Synergistic effect of intrathecal fentanyl and bupivacaine in spinal anesthesia for cesarean section

BACKGROUND: Potentiating the effect of intrathecal local anesthetics by addition of intrathecal opiods for intra-abdominal surgeries is known. In this study by addition of fentanyl we tried to minimize the dose of bupivacaine, thereby reducing the side effects caused by higher doses of intrathecal bupivacaine in cesarean section. METHODS: Study was performed on 120 cesarean section parturients ...

MINIDOSE BUPIVACAINE – FENTANYL SPINAL ANESTHESIA FOR CESAREAN SECTION IN PREECLAMPTIC PARTURIENTS

ABSTRACT Background: Spinal anesthesia for cesarean section is associated with a high incidence of hypotension. The synergism between intrathecal opioids and local anesthetics may make it possible to achieve reliable spinal anesthesia with minimal hypotension using a minidose of local anesthetic. Methods: Forty-four preeclamptic parturients undergoing cesarean section were randomized in two gr...

Intrathecal fentanyl for prevention of shivering in spinal anesthesia in cesarean section

Background: Shivering is one of the common problems in spinal anesthesia. The objective of the present study was to evaluate the effect of intrathecal fentanyl (25 μg) on incidence and severity of intraoperative and postoperative shivering. Methods: A double-blind randomized controlled study was conducted in eighty healthy women (ASA Physical status I) scheduled for elective cesarean section u...
